Dear Parents:

The following is a Parent SELF evaluation form, designed to give us the ability to review our own behaviors at sporting events. This evaluation form reflects the CYO philosophy and will help shape the St Francis programs. Please work to be honest with yourself and take the time to do this brief evaluation.

The information you provide is STRICTLY CONFIDENTIAL and will only be seen by the Athletic Director.

Please answer the questions accordingly......10 = Strongly Agree - 1 = Strongly Disagree

1) Do you prioritize arriving with your child at the practices and competitions on time, prepared to perform, and encourage them to fulill the commitment they made to the team? :
2) Do you teach a team first mentality to your child? :
3) If you were your child, would you feel supported and encouraged by your words and actions that pertain to your athletic performance? :
4) Is your behavior as a parent fan, an example that you would be proud to see in your own child? :
5) Are you more likely to cheer FOR your team as opposed to AGAINST the other team? :
6) Do you typically leave a competition feeling satisfied with the referee, umpire or other officiating? :
7) As a parent fan do you refrain from coaching your child's team from the stands and let the coach, coach? :
8) Are you able to curb your desire to verbally criticize the decisions of the officials, the coach, the players or your child during or immediately following the competition? :
9) Do you "Let the water settle" and wait 24 hours before discussing the competition with your child or the coach? :
